Family Practice APRN - Clinic
Job Summary
We are seeking a dedicated and skilled Advanced Practice Registered Nurse (APRN) to join our family practice team. The ideal candidate will have experience in family practice settings, particularly in a rural setting, and possess a strong foundation in nursing practices. This role involves providing high-quality patient care, collaborating with multidisciplinary teams, and ensuring optimal health outcomes for patients across various specialties, including pediatrics and geriatrics.
Responsibilities
 Conduct comprehensive patient assessments and develop individualized care plans.
Collaborate with physicians and other healthcare professionals to coordinate patient care.
Participate in discharge planning to ensure smooth transitions for patients leaving the facility.
Provide education and support to patients and families regarding treatment plans and health management.
Document patient interactions and care provided using eClinicalWorks or similar electronic health record systems.
Maintain up-to-date knowledge of best practices in nursing and participate in ongoing professional development.
Skills
Strong clinical skills in family practice nursing..
Experience working with children as well as geriatrics and understanding their unique healthcare needs.
Familiarity with eClinicalWorks or similar electronic health record systems for efficient documentation.
Excellent communication skills to effectively educate patients and collaborate with healthcare teams.
Ability to engage in discharge planning processes to facilitate patient transitions from hospital to home or other care settings.
